Before your child enters the Sixth Grade in September, 2017, he/she is required to receive the Tdap Immunization. This requirement is for any student who is 11 years or older.
If your child is under 11 years, he/she will be allowed to enter Middle School but his/her health record will be tracked.
Additionally, your child will need are two Varicella Immunizations and three to four Polio Immunizations with the last one on or after the fourth birthday. These are required for the start of sixth grade.
Please submit to me written documentation from your healthcare provider that the immunization has been given OR submit a written statement from your healthcare provider of the scheduled appointment date to receive the immunization.
